Use this form when you need to dispute a check/draft due to forgery.

ATM/POS Transaction Inquiry Form

Complete this form when you need to inquire about a transaction from an ATM machine or point-of-sale (POS) Debit transaction.

Authorization For Loan Payment ACH/Debit

Complete this form to authorize TEG to automatically debit your specified account to make TEG loan payments.

Authorization for Loan Payment Change

Complete this form to authorize changes to your loan payment method or monthly payment amount.

Unauthorized Debit Dispute Form

Use this form when you need to dispute a charge to your account by debit or ACH.

Wire Transfer Form

Complete this form to initiate a wire transfer. Please note that if this is your first time doing a wire transfer with TEG you must come into a branch to create a password prior to requesting a wire transfer over the phone or via fax. Subsequent wire transfer requests can be done by completing this form with your password and faxing it to the credit union at (845) 790-6624 or calling us at (845) 452-7323.
